V12Waiver V12Waiver Parents Name * Athletes Name * Team Name PLEASE READ CAREFULLY! THIS IS A LEGAL DOCUMENT THAT AFFECTS YOUR LEGAL RIGHTS! This Release and Waiver of Liability (the “Release”) executed on this ____ day of __________, 2021 by ____________________, a minor child (the “Volunteer”), and _____________________, the parent having legal custody and/or the legal guardian of the Volunteer (the “Guardian”), releases V12 Athletics (V12 Invitational/Coach Miami Presents’ Events) an organization that exists under the laws of the State of Florida and each of its directors, officers, employees, and agents. The Volunteer and Guardian desire that the Volunteer engage in activities related to serving as a volunteer of V12 Athletics/Coach Miami Presents’ Events. The Volunteer is responsible for his/her own insurance coverage in the event of personal injury or illness as a result of Volunteer’s services to the V12 Athletics/Coach Miami Presents’ Events. 1. Waiver and Release: We, the Volunteer and the Guardian, release and forever discharge and hold harmless AGR and its successors and assigns from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from the services the Volunteer provides to V12 Athletics/Coach Miami Presents’ Events. We understand and acknowledge that this Release discharges V12 Invitational from any liability or claim that we may have against V12 Athletics/Coach Miami Presents’ Events with respect to bodily injury, personal injury, illness, death, or property damage that may result from the services the Volunteer provides to V12 Athletics/Coach Miami Presents’ Events or occurring while Volunteer is providing volunteer services. 2. Insurance: Further, we understand that V12 Athletics/Coach Miami Presents’ Events does not assume any responsibility for or obligation to provide me with financial or other assistance, including but not limited to medical, health or disability benefits or insurance of any nature in the event of my injury, illness, death or damage to my property. I expressly waive any such claim for compensation or liability on the part of V12 Athletics/Coach Miami Presents’ Events beyond what may be offered freely by V12 Athletics/Coach Miami Presents’ Events in the event of such injury or medical expenses incurred by me. 3. Medical Treatment: I hereby release and forever discharge V12 Athletics/Coach Miami Presents’ Events from any claim whatsoever which arises or may hereafter arise on account of any first-aid treatment or other medical services rendered in connection with an emergency during my tenure as a volunteer with V12 Athletics/Coach Miami Presents’ Events. 4. Assumption of Risk: As a volunteer, I hereby expressly assume the risk of injury or harm from these activities and release V12 Athletics/Coach Miami Presents’ Events from all liability for injury, illness, death, or property damage resulting from the services I provide as a volunteer or occurring while I am providing volunteer services By checking the box and typing or signing my name below, I express my understanding and intent to enter into this Release and Waiver of Liability willingly and voluntarily. Under the conditions in which our country is in due to the COVID-19 virus, this waiver holds true for any occurrences abused by not practicing the social distance mandates currently in place to secure the health and safety for everyone participating in this event. We, the V12 Athletics/Coach Miami Presents staff are not responsible for any unfortunate illness or injury that may occur while participating in the V12 Summer Games and by signing this waiver you agree to these terms and conditions.
